After your tour, you’ll receive our enrollment packet. These forms collect important details such as health information, routines, and any special requirements, so we can ensure your child’s transition into our care is seamless and personalized.

Submit and Secure
To finalize enrollment, return the completed packet with the required registration fee. This officially secures your child’s place in our program. Once received, our team will confirm your start date and welcome you to the Carepointe family.

We’re here to support your busy schedule. Both Fort Wayne campuses are open Monday through Friday, 6:00 AM to 5:30 PM, providing flexibility and peace of mind for working families.
